0nline appointments to see our Physio will be available to book online from Monday 2nd December, you need to have patient access to use this service.
What is a first contact physio appointment? 
First Contact Practitioners (FCPs) are physiotherapists usually based in GP surgeries. They can assess and diagnose what’s happening, Give expert advice on how best to manage your condition and refer on to specialist services if necessary.

Statement of intent on information technology

Patient online services giving you access to appointments, prescriptions, investigations (blood test results) and a summary of your medical records

Our practice promotes and offers to its registered patients the facility for a patient:

  • To book, view, amend, cancel and print appointments online;
  • To order repeat prescriptions for drugs, medicines or appliances online; and
  • To view and print a list of any drugs, medicines or appliances in respect of which the patient has a repeat prescription
    This facility is electronically integrated with the computerised clinical systems of the practice using appropriate authorised systems.

Our practice promotes and offers to its registered patients the facility for a patient to:

  • Access online any summary information derived from the patient’s medical records and other data such as investigations (blood test results) which our practice has agreed that the patient may access; and
  • View online, electronically export or print any summary information derived from the patient’s medical records and any other data which our practice has agreed that the patient may access.

Your summary care record updated daily

Our practice in any case where there is a change to the information included in a patient’s medical record, has enabled an automated upload of summary information to the Summary Care Record, at least on a daily basis, using approved automated systems.

Clinical correspondence and your NHS number

Our practice includes the NHS number of a registered patient as the primary identifier in all clinical correspondence issued by our practice. You can find your NHS number using the NHS choices link.

Electronic transfer of your patient records completed efficiently

Our practice uses the electronic GP2GP facility for the safe and effective transfer of any patient records:

  • In a case where a new patient registers with our practice, to move the patient record from the practice with which the patient was previously registered; or
  • In a case where our practice receives a request from another practice with which the patient has registered, in order to respond to that request.
